System Outages

While I was in Oregon, there was huge system outage at our data center. Although, it was triggered by upgrading one of he server firmware ... it is totally our responsibility. I are currently figuring out;
(1) What has happened?
(2) Why it happened?
(3) What can we do to avoid the same problem.
We are very sorry for what happened ... We are working hard to make sure the same thing will not happen again.
